Home Help

This is also known as Chores Services. Home Help is a Michigan Medicaid-funded program designed to provide assistance with daily living activities for individuals who need help due to a physical disability, chronic illness, or age-related condition. Administered by the Michigan Department of Health and Human Services (MDHHS), this program helps eligible individuals maintain their independence by receiving necessary care and support in their own homes or community settings, rather than institutional care.

Home Help services are primarily aimed at assisting individuals with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living(IADLs) to ensure their safety, well-being, and quality of life. The Process of Receiving Home Help Services
Step 1: Medicaid Enrollment:- The individual must be enrolled in Michigan Medicaid.
Step 2: Application: The individual or their guardian applies for the Home Help program through their local MDHHS office. Download, complete and submit the application forms (Form DHS-360) and Form DHS-54-A (to be completed and signed by applicant’s Physician). Then submit both completed forms to the nearest MDHHS office close to your residence.
Step 3: Assessment: An MDHHS caseworker visits the individual’s home to assess their needs and complete the Functional Assessment Instrument (FAI).
The assessment includes an evaluation of the individual’s ability to perform ADLs and IADLs.
Step 4: Development of a Care Plan:
Based on the assessment, a care plan is developed to outline the specific services the individual requires, the frequency of services, and the expected outcomes.
Step 5: Service Authorization: The care plan is submitted for approval, and Medicaid authorizes funding for the services.
Step 6: Selection of Caregiver: The individual can choose a caregiver, such as a family member, friend, or professional caregiver from a home care agency, to provide Home Help services.
Caregivers must meet specific eligibility and training requirements and pass a background check.
Step 7: Service Delivery and Monitoring: The caregiver provides services according to the approved care plan. MDHHS regularly monitors the quality and appropriateness of services through follow-up visits and communication with the client.
